RN Case Manager

About This Role

The RN Case Manager provides skilled nursing care to terminally ill patients, conducting ongoing assessments and collaborating with others to create and implement individualized care plans. This role focuses on optimizing patient outcomes through effective case management and patient education.

Responsibilities

• Provide skilled nursing care to terminally ill patients based on ongoing nursing assessment
• Plan, implement, and evaluate nursing care through systematic assessment of health needs
• Utilize clinical tools, data, and other information to complete patient needs assessments
• Create individualized and comprehensive case management and treatment plans
• Educate patients and caregivers on benefits, services, and other resources
• Document complex medical information to outline and improve results
• Follow case and quality management processes in compliance with regulatory guidelines and company procedures
• Assess and respond to patient and family needs in a timely manner
• Establish professional and collaborative relationships with patients, families, and providers
• Provide care and assistance to patients and families during on-call hours as scheduled

Requirements

• Current multi-state RN license in Kentucky or Ohio
• Valid driver's license
• Auto insurance
• Ability to access patient homes and facilities

Qualifications

• Graduate of an accredited School of Nursing
• At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility is preferred.
