The RN Case Manager coordinates and facilitates patient care through collaboration with the Interdisciplinary Care Transitions team. This role ensures optimal resource utilization, service delivery, and compliance with external review agencies. They provide comprehensive assessment, care planning, and evaluation of patient needs to enhance patient management and promote continuity of care and cost-effectiveness.
Responsibilities
- Assist in coordinating clinical and/or psycho-social activities with the Interdisciplinary Team and Physicians
- Assist with effective care coordination and efficient care facilitation
- Remain current regarding reimbursement modalities, community resources, case management, psychosocial and legal issues
- Appropriately refer high-risk patients who would benefit from additional support
- Serve as a patient advocate
- Participate in interdisciplinary patient care rounds and/or conferences
- Collaborate with clinical staff in the execution of the plan of care and achievement of goals
Requirements
- Healthcare professional licensure (LPN/LVN, RN, LCSW, or LSW)
- Knowledge of government and non-government payor practices, regulations, standards and reimbursement
- Knowledge of Medicare benefits and insurance processes and contracts
- Knowledge of accreditation standards and compliance requirements
- Fluent in English
- Basic computer skills with working knowledge of Microsoft Office
Qualifications
- Graduate of an accredited LPN/LVN or RN program; Master of Social Work with licensure or Bachelor of Social Work with licensure as required by state regulations
- One year of experience in a healthcare setting
