RN Case Manager, Inpatient

About This Role

The Inpatient RN Case Manager at Boston Children's Hospital coordinates patient transitions from hospital to home or facilities, considering clinical needs and insurance options. This role involves performing clinical assessments and collaborating with multidisciplinary teams to ensure patients have necessary equipment and services for safe discharge.

Responsibilities

• Assess, plan, organize and oversee daily clinical operations of care coordination and transitions to address comprehensive and complex health needs of the patient and family
• Possess knowledge of community health services, payer systems and post-acute care needs for inpatient patients and families
• Collaborate with inpatient nursing teams and inpatient case managers
• Demonstrate the ability to integrate regulatory requirements, length of stay, resource allocation, funding sources, and capacity management to optimize delivery of appropriate services
• Actively participate in hospital, departmental and multidisciplinary programs, committees, and special projects
• Work 40 hours per week, Monday through Friday, 8:00 am to 4:30 pm with rotating holidays and weekends

Requirements

• Bachelor's degree in nursing
• 5 years nursing experience
• Current Massachusetts Registered Nurse (RN) license

Qualifications

• A Bachelor's degree in nursing
• A minimum of 5 years nursing experience required; pediatrics preferred. Experience as a Case Manager preferred.

Nice to Have

• Master's in a related Healthcare field
• Pediatrics experience
• Experience as a Case Manager (Discharge Planning and/or Utilization Management)
• Certification as a Certified Case Manager or willingness to obtain within two years
