The RN Case Manager coordinates with physicians, nurses, and other health team members to ensure medically appropriate and cost-effective patient care. This role applies clinical expertise to resource utilization and discharge planning, facilitating patient stays from admission to discharge.
Responsibilities
- Coordinate with physicians, nurses, social workers and other health team members to expedite medically appropriate, cost-effective care
- Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning
- Facilitate resource management activities for inpatients based on identified patients
- Coordinate patient’s stay from admission to discharge through collaboration with healthcare professionals
- Enhance continuity of patient care, promote cost effective utilization of resources, and enhance patient, nurse and physician satisfaction
- Provide psychosocial assessment, resources and intervention to hospitalized patients
Requirements
- Current Alabama or Multistate Nursing License
- BLS for Healthcare Providers required by the end of orientation period
- ACLS required within 1 year of hire
- Effective communication skills, verbal and written
- Excellent organization skills
Qualifications
- Graduate of accredited School of Nursing
- Three years clinical experience strongly preferred, or comparable combination of education and experience. Previous utilization management, discharge planning or hospital based social service experience is very beneficial.
