Join St. Luke's Home Health Team as a Home Health Case Manager, collaborating with physicians and multidisciplinary teams to create and manage individualized care plans for patients. This direct care role involves traveling within Boise, ID and surrounding regions to deliver in-home care, ensuring the highest standard of care from admission to discharge.
Responsibilities
- Collaborate with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload
- Deliver in-home care from admission to discharge within Boise, ID and surrounding regions
- Ensure each patient receives the highest standard of care, aligned with their needs and the care plan
Requirements
- Current RN license in the state of Idaho
- Basic Life Support (BLS) certification
- Valid driver’s license
- Proof of auto insurance
Qualifications
- Nursing Degree (ASN or BSN)
- 2 years of RN experience; Home Health or Acute Care experience preferred
