Provide compassionate PRN care to terminally ill patients in their homes, focusing on symptom management, medication oversight, and emotional and spiritual support. Collaborate with an interdisciplinary team to develop and manage personalized care plans.
Responsibilities
- Provide PRN care to terminally ill patients in their homes
- Conduct admission and informational visits for patients
- Manage imminent and some routine visits for patients
- Collaborate with the interdisciplinary team (IDT) on care plan development and management
- Focus on symptom management and medication oversight
- Provide emotional and spiritual support to patients and families
- Educate patients and families about the care process
Requirements
- Nursing Degree (ASN or BSN)
- At least 1 year of RN experience
- Current RN license in the state of Idaho
- Basic Life Support (BLS) certification
- Valid driver’s license
- Proof of auto insurance
Qualifications
