About This Role

The Registered Nurse provides professional care for patients, assessing, planning, implementing, and evaluating individual patient care in a nursing department. This role involves supervising co-workers and adhering to Nursing Services departmental policies and standards of care.

Responsibilities

  • Assess patient’s condition, especially pain and symptoms, and initiate appropriate nursing and/or medical care in accordance with established guidelines
  • Develop a plan of care based upon the assessment of the Hospice patient and in congruence with the medical diagnosis and treatment plan
  • Document assessment, plan of care, and patient response in the patient’s record
  • Communicate significant information and changes in patient's condition to appropriate members of the Hospice Interdisciplinary team
  • Delegate, direct, and supervise implementation of care plan to LPN, Home Health Aides / PCTs, or other members of the Interdisciplinary Team (IDT)
  • Educate patient/family according to assessed need and communicate Hospice philosophy and program
  • Assume the mentor role for new Associates and students
  • Participate in bereavement assessment and bereavement activities as appropriate

Requirements

  • Current Michigan license to practice as Registered Nurse
  • Active American Heart Association BLS Certification within 60 days of hire
  • Current Driver’s License
  • Current Proof of Auto Insurance
  • A minimum of three full time years’ experience in terminal care or two full time years Hospice experience
  • Associate degree in nursing
  • Computer literate
  • Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans and deliver appropriate care
  • Interpersonal skills to effectively and appropriately communicate with patients, families, physician, other Sparrow Regional Health System Associates and external agencies

Qualifications

  • Associate degree in nursing
  • A minimum of three full time years’ experience in terminal care or two full time years Hospice experience.

Nice to Have

  • Hospice certification
  • BSN from an accredited institution
  • Mckesson documentation experience
