The Registered Nurse provides professional care for patients, assessing, planning, implementing, and evaluating individual patient care in a nursing department. This role involves supervising co-workers and adhering to Nursing Services departmental policies and standards of care.
Responsibilities
- Assess patient’s condition, especially pain and symptoms, and initiate appropriate nursing and/or medical care in accordance with established guidelines
- Develop a plan of care based upon the assessment of the Hospice patient and in congruence with the medical diagnosis and treatment plan
- Document assessment, plan of care, and patient response in the patient’s record
- Communicate significant information and changes in patient's condition to appropriate members of the Hospice Interdisciplinary team
- Delegate, direct, and supervise implementation of care plan to LPN, Home Health Aides / PCTs, or other members of the Interdisciplinary Team (IDT)
- Educate patient/family according to assessed need and communicate Hospice philosophy and program
- Assume the mentor role for new Associates and students
- Participate in bereavement assessment and bereavement activities as appropriate
Requirements
- Current Michigan license to practice as Registered Nurse
- Active American Heart Association BLS Certification within 60 days of hire
- Current Driver’s License
- Current Proof of Auto Insurance
- A minimum of three full time years’ experience in terminal care or two full time years Hospice experience
- Associate degree in nursing
- Computer literate
- Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans and deliver appropriate care
- Interpersonal skills to effectively and appropriately communicate with patients, families, physician, other Sparrow Regional Health System Associates and external agencies
Qualifications
- Associate degree in nursing
- A minimum of three full time years’ experience in terminal care or two full time years Hospice experience.
Nice to Have
- Hospice certification
- BSN from an accredited institution
- Mckesson documentation experience
