About This Role

Join a quality-focused interdisciplinary care team to provide 1:1 skilled nursing and rehabilitative care to home health clients in Yavapai County, ensuring they can remain at home.

Responsibilities

• Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plans
• Administer medication, wound care, and treatments
• Perform client assessments, provide education to patients and/or their families, and monitor patients' conditions
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
• Participate in weekend on-call rotation

Requirements

• Current nursing license in good standing in the state of Arizona
• Graduation from an accredited and approved nursing program
• 1 year nursing experience
• Prior home health experience
• OASIS experience

Qualifications

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• 1 year nursing experience
