Registered Nurse Case Manager

About This Role

Serve as a Registered Nurse Case Manager for hospice patients, assessing physical, psychosocial, and environmental needs, and communicating findings to the interdisciplinary team. Instruct patients, families, and staff on hospice philosophy and pain management.

Responsibilities

• Ensure all activities align with Elara Caring’s vision and the Hospice team’s leadership
• Assess hospice care patients and families to identify physical, psychosocial, and environmental needs
• Obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status
• Communicate significant findings, problems, and changes in health condition to the Clinical Supervisor, physician, and other personnel
• Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care
• Teach hospice philosophy and pain and symptom management to staff in alternate care settings
• Provide appropriate support at time of death and perform bereavement assessment

Requirements

• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Valid driver’s license and insurance
• Reliable transportation

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1 year of experience in a clinical care setting

Nice to Have

• Experience in a hospice or home health environment
