About This Role

This role involves assessing patients' psychosocial, financial, and environmental needs, developing and implementing individualized plans of care, and communicating findings to the healthcare team to ensure comprehensive patient support in a home care setting.

Responsibilities

  • Ensure all activities align with Elara Caring's vision and leadership.
  • Assess patients' psychosocial, financial, and environmental needs through documentation, clinical records, and evaluations.
  • Make initial social work evaluation visits and reevaluate patient needs during subsequent visits.
  • Communicate significant findings, problems, and changes in condition or environment to the Supervisor, physician, and other personnel.
  • Report unsafe conditions and visit outcomes to the appropriate Supervisor by the end of the day.
  • Implement plans for patient safety using patient, family, and community resources.
  • Participate in the implementation and development of the Plan of Care for quality, continuity, and discharge planning.
  • Verify and provide care according to the Plan of Care, physician's orders, assessment data, and established standards.
  • Initiate and revise the Plan of Care in response to identified patient care issues.
  • Write physician orders for additional visits and changes to the plan of care.
  • Incorporate patient care goals into clinical notes.
  • Perform appropriate skilled services/interventions in accordance with accepted standards and physician certification.
  • Counsel, instruct, and involve patients and families in following the Plan of Care and meeting social work-related needs.

Requirements

  • Master’s Degree or Doctoral Degree in Social Work from an accredited school
  • 1+ year of social work experience in a healthcare setting
  • Current State License as a Social Worker
  • Excellent verbal and written communication skills
  • Social work skills as defined by general social work standards
  • Reliable transportation and insurance

Qualifications

  • Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
  • 1+ year of social work experience in a healthcare setting.
