This role involves assessing patient needs and developing care plans, conducting initial and follow-up evaluations and communicating patient status to an interdisciplinary team. The Registered Nurse will also educate staff on hospice philosophy and pain management.
Responsibilities
- Assess patient needs and obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status; incorporate data into plan of care
- Conduct initial evaluation and re-evaluate patient’s nursing needs during each visit, making follow-up visits as needed
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel
- Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members
- Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care
- Teach hospice philosophy and pain and symptom management to staff in alternate care settings
- Collaborate/Communicate with Medical Director, attending physician, branch leadership and IDG
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State Licensee as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Nursing skills as defined by accepted nursing standards
- Ability to handle multiple priorities, documentation requirements, and deadlines
- Able and willing to travel up to 50% as needed to support business and patient needs
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs
- Valid driver’s license and insurance and reliable transportation
Qualifications
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- 1 year of experience in a clinical care setting
