This role involves providing hands-on care and emotional support to senior residents, while also managing medication administration to ensure their well-being in a safe and comfortable environment.
Responsibilities
- Designate a group of residents during the shift, tracking their location and performing physical checks
- Observe, report, and document changes in resident conditions, including skin, behavior, alertness, weight, dietary intake, and activity participation
- Respond promptly to security system alerts and resident call bells
- Notify supervisor or Health Care Coordinator of increased resident care needs
- Assist with continence management and dispose of products properly to maintain sanitation
- Serve and collect food trays, assisting residents as needed and reporting refusal of meals or loss of appetite
- Maintain cleanliness of resident's room and work areas, practicing standard care precautions
- Help residents maintain independence, promoting dignity and physical safety
- Actively participate/lead and assist residents with activities of daily living (bathing, dressing, toileting, grooming, ambulation, transferring, eating)
- Engage residents in life skills and other life enrichment activities
- Greet and assist all internal and external customers, guests, family members, residents, vendors, and team members
- Do resident laundry as assigned
- Review service plans to learn pertinent information about residents
- Assist/observe medications and treatments for each resident using the medication observation record and the Six Rights of Medication Pass
- Document and initial medication administration, including appropriate documentation for refusal or missed doses
- Maintain confidentiality of all resident medication information
- Report all resident concerns made while assisting with medication to the Nurse and/or Health Care Coordinator (HCC)
- Restock medication cart after all medication passes
- Assist in checking medication regardless of packaging system
- Count all narcotics with another Medication Care Manager or Nurse each shift
- Maintain and clean the Medication Room, med charts, and treatment carts for neatness, cleanliness, and availability of medications, checking for expired medications
- Follow re-fill process for medications
- Participate in the development of the Service Plan and monthly updates
- Take and record temperature, blood pressure, weight, pulse, and respiration rates
- Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately
Requirements
- High School diploma/GED
- Must be 18 years of age
Qualifications
- High School diploma/GED
- Previous experience working with seniors preferred.
Nice to Have
- Certified Nurse’s Aid certification
- CPR Certification
- First Aid Certification
- Previous experience working with seniors
