LVN - Floor Nurse

About This Role

Join a senior living community as an LVN, participating in the development and maintenance of patient treatment plans and ensuring high-quality patient care in accordance with agency standards.

Responsibilities

• Participate in the development and maintenance of treatment plans
• Assist with the development, implementation, and communication of patient plans of care
• Communicate identified patient needs to the Staff Registered Nurse and/or Clinical Services Coordinator consistently and in a timely manner
• Notify Staff Registered Nurse and/or Clinical Services Coordinator immediately of any significant changes in the patient’s condition
• Participate in case conferences as per Agency policies
• Involve patients and family members in developing and implementing the plan of care whenever possible
• Provide information regarding patient rights and responsibilities and treatment plans to patients and/or family members
• Respond to and communicate patient questions, concerns, and/or complaints in a timely and professional manner
• Implement current nursing practice
• Assess patient conditions in every home health care visit
• Implement skilled nursing interventions to meet patient needs and changed conditions
• Maintain accurate, complete and timely clinical records
• Implement appropriate infection control and safety measures and report all hazards and accidents in a timely and accurate manner
• Maintain confidentiality of patient information
• Participate in professional growth and development

Requirements

• Current professional license and remains in good standing (LVN)
