Conduct in-home health assessments for adult patients as a licensed Nurse Practitioner or Physician Assistant. This is a per diem, flexible opportunity to supplement income while maintaining control over your schedule.
Responsibilities
- Conduct comprehensive medical histories and physical exams
- Evaluate social determinants of health
- Order and review diagnostics (as applicable)
- Develop, initiate, and adjust treatment plans
- Manage and reconcile medications (as applicable)
- Educate patients on disease prevention, medication use, lifestyle modifications, self-care, and wellness
- Complete and submit visit documentation within 24 hours (typically completed during the visit)
Requirements
- Active NP or PA license
- Minimum 1 year of clinical experience
- Ability to commit to at least 30 hours per month
- Comfortable working independently in home-based settings
