Primary Care Physician

About This Role

This role is for a Board Certified/Eligible Primary Care Physician to provide comprehensive outpatient care to Veterans within a federal healthcare system in the Finger Lakes region. The physician will lead a Patient Aligned Care Team, managing a balanced patient panel with a focus on continuity and quality of care.

Responsibilities

• Provide primary care services to an average of 12–13 patients per day, including acute, new, and routine visits
• Perform comprehensive histories and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Develop individualized treatment plans
• Complete all clinical documentation using CPRS
• Close all patient encounters by the end of each shift
• Utilize VistA Imaging for clinical documentation and order entry
• Direct ongoing outpatient care
• Conduct preventive care, health education, and disease prevention activities

Requirements

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Current, full, and unrestricted medical license in any U.S. state or territory
• Active BLS and ACLS certification
• Active DEA registration
• National Provider Identifier (NPI)
• Fluency in spoken and written English
• Proficiency with CPRS and VistA Imaging
• Ability to manage diverse patient populations and complex medical conditions
• Strong diagnostic and communication skills, including timely follow-up of test results
