Join Platinum Home Health Services as a Physical Therapist (PT) to provide high-quality, patient-centered care, helping patients improve mobility, function, and overall quality of life in their homes. You will perform comprehensive evaluations, develop individualized plans, and implement therapeutic interventions to support patient well-being.
Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized plans of care
- Provide skilled physical therapy services in accordance with physician orders and agency policies
- Assess patient mobility, strength, balance, and functional limitations
- Implement therapeutic exercises, gait training, and pain-management techniques
- Educate patients and caregivers on treatment plans, safety, and home exercise programs
- Document evaluations, progress notes, and discharge summaries accurately and timely
- Communicate effectively with physicians, case managers, and interdisciplinary team members
- Follow all state, federal, and Medicare home health regulations
Requirements
- Current Physical Therapist license in the State of Ohio
- Graduate of an accredited Physical Therapy program
- Strong clinical, communication, and organizational skills
- Ability to work independently and manage a caseload
- Valid driver ’s license, reliable transportation, and auto insurance
Qualifications
- Graduate of an accredited Physical Therapy program
