About This Role

Manage and provide skilled physical therapy to patients with diverse diagnoses and disabilities in a home setting, collaborating with physicians to achieve maximum patient independence tailored to individual needs.

Responsibilities

• Manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting
• Adhere to a physician's plan of treatment
• Take into account individual patients' special or age related needs
• Perform patient/client evaluation and treatment
• Provide professional consultations to achieve maximum patient/client independence

Requirements

• Bachelor's Degree or Master's Degree in Physical Therapy from an accredited institution
• Oregon Physical Therapist License upon hire
• National Provider BLS - American Heart Association within 30 days of hire
• 1 year of clinical experience in a healthcare setting (preferably home health) or active enrollment in the Providence Benedictine postgraduate preceptor program

Qualifications

• Bachelor's Degree in Physical Therapy or Master's Degree in Physical Therapy from an accredited institution
• 1 year Clinical experience in a healthcare setting, preferably home health, or active enrollment in the Providence Benedictine postgraduate preceptor program.
