Physical Therapist, Home Health

About This Role

This role involves providing physical therapy services to patients in their homes, focusing on maintaining, restoring, and improving movement and function. Join a large home health and hospice company dedicated to patient care and work-life balance.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function
• Provide interventions and conduct reexaminations (every 30 days, at minimum)
• Modify interventions as necessary
• Perform discharge planning

Requirements

• Current license to practice physical therapy specific to the state
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE)
• One year related physical therapy experience for PTs entering home health care (or approval for less)
• Current CPR certification

Qualifications

• Graduate from an accredited physical therapy educational program (CAPTE)
• One year related physical therapy experience for PTs entering home health care, or approval from regional clinical leaders and mentoring/residency for less than one year of experience
