About This Role

The Physical Therapist Assistant at H2 Health provides 1:1 patient care within luxury Senior Living communities in Jacksonville, FL, focusing on individualized therapy and patient advocacy with a flexible part-time schedule.

Responsibilities

Provide 1:1 patient care to residents within Senior Living communities
Serve as a provider and extension of the communities served
Collaborate with an interdisciplinary team to deliver therapy opportunities

Requirements

Graduate of an accredited Physical Therapist Assistant program
Active or eligible state license to practice

Qualifications

Graduate of an accredited Physical Therapist Assistant program
Prior experience preferred, new graduates welcome

Nice to Have

Prior experience as a Physical Therapist Assistant
