This Per Diem Pharmacist role at Coastal Family Health in Astoria, OR, involves providing comprehensive patient care services and clinical support, ensuring the delivery of cost-effective medications and optimal care.
Responsibilities
- Provide patient care services including Drug Utilization Review, Patient Counseling, and Medication Therapy Management
- Perform Collaborative Drug Therapy Management or other post-diagnostic disease state management
- Prescribe drugs or devices as authorized by statute and protocols
- Order, interpret, and monitor laboratory tests
- Receive or transfer prescriptions
- Provide direct oversight of pharmacy personnel, ensuring work is within their scope of practice
- Ensure pharmacy area security and safeguard against theft or diversion of prescription drugs
- Maintain records and inventories in accordance with state and federal laws
- Ensure correct dispensing of new prescriptions, refills, and drug orders
- Maintain active licensure and pharmacological knowledge through educational activities and professional participation
Requirements
- Bachelor's Degree in Pharmacy
- Current Oregon State Pharmacist License

- Bachelor's Degree in Pharmacy

- Bilingual (English/Spanish) speaking skills
- Experience in community or ambulatory pharmacy practice
- PGY-1 & PGY-2 residencies
