The Patient Navigator role supports Strive Health's care team by engaging patients with kidney conditions in their complete care program. This involves proactive outreach, relationship building, education, and scheduling appointments to improve patient outcomes.
Responsibilities
- Proactively outreach to current and prospective patients via phone to educate them on care services and enroll them within an assigned geographic area
- Build trusting, long-term relationships with patients and appropriately educate on enrollment requirements
- Meet or exceed daily outreach expectations for outbound calls, connections, and enrollments
- Present as an educational and empathetic resource to potential patients about the care model
- Build key internal relationships to escalate patient support needs
- Visit patients in-person, when possible, at home or during medical visits to schedule future appointments
- Conduct face-to-face patient outreach including at home door knocking, clinic visits, or events
- Meet with partner nephrologists and practice staff to build Strive's credibility
- Maintain up-to-date client information in CRM tool (i.e., Salesforce)
- Maintain long-term relationships with patients to support retention and satisfaction
Requirements
- High School Diploma
- 5+ years combined education or experience working with customers or patients in a professional setting
- Efficient and reliable transportation, including an active driver’s license
- Ability to travel across an assigned region to meet patient needs (offices, clinics, patient homes)
- Ability to provide in-person patient care including standing, sitting, walking, pushing, pulling, and lifting
- Minimum internet speeds: 3.8Mbps/3.0Mbps (up/down)
Qualifications
- High School Diploma
- 5+ years combined education or experience working with customers or patients in a professional setting
Nice to Have
- Bachelor’s Degree in Business Administration or related
- Sales background
- Experience with promoting and selling primary care or other healthcare related services to high needs populations
- Experience in healthcare, specifically educating patients on care models or insurance plans
- Community health experience
- Proficient PC skills
- Proficient computer skills
