This role executes the Kidney Navigator program for patients requiring dialysis at acute or hospital discharge, focusing on education, referrals, and engagement to transition patients to longitudinal management and enhance their experience through shared decision-making.
Responsibilities
- Meet with patients requiring outpatient dialysis placement or for chronic kidney disease (CKD) education as defined by program
- Educate patients on the progression of disease for CKD, transplants, dialysis, modality options, access options, and insurance
- Demonstrate empathy for individuals based on a thorough understanding of patients’ individual needs
- Provide a hand-off to Strive’s Enrollment Associates, sharing patient’s unique situation and ongoing needs through our care management platform
- Provide quality hand-off to departments post-discharge program if the patient chooses not to enroll
Requirements
- 2+ years combined of related education, experience, or certification
- Efficient and reliable transportation allowing for the ability to commute to patient homes or hospitals
- Ability to provide in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency
Qualifications
- 2+ years combined of related education, experience, or certification
Nice to Have
- Licensed LPN/Social Worker/Dialysis Technician
- 1+ years experience in dialysis setting
- Healthcare experience
- Insurance knowledge
- Case management experience
