About This Role

This role is a full-time Certified Nursing Assistant (CNA) position within a community-based PACE program, responsible for coordinating unskilled home care services and conducting in-home safety evaluations. The coordinator supports care delivery for frail, medically complex seniors living independently in the community.

Responsibilities

Coordinate unskilled home care services
Conduct in-home safety evaluations
Support care delivery for frail, medically complex seniors
Coordinate services like meal deliveries, caregiver visits, and medical equipment drop-offs
Advocate for participant needs
Ensure services align with care goals
Collaborate closely with nurses, social workers, and other team members

Requirements

Active CNA and California Home Health Aide (HHA) certification
CPR and First Aid certification from the American Heart Association
1+ year of experience working with frail or elderly adults
Valid California driver’s license
Basic computer skills (Microsoft Outlook, Excel)
