Turning Point Community Programs is hiring an Outreach Worker for the Core-Rosin program in Sacramento, CA. This role focuses on providing integrated mental health services and support to homeless individuals and program members through community outreach and case management.
Responsibilities
- Dedicate 30 hours per week to street and community outreach at various locations where homeless individuals spend time.
- Reconnect with members who have lost contact with staff for 30 or more days.
- Provide follow-up and continued outreach to members who have voluntarily discharged from the program and remain at risk.
- Maintain a continuous outreach caseload of 15 non-enrolled individuals.
- Maintain a continuous enrolled caseload of between 3-5 members as needed.
- Make no less than 30 face-to-face contacts per week with individuals who are the focus of outreach services.
- Assist assigned clients in identifying and progressing toward achievement of their chosen goals.
- Advocate for and assist clients and their families in accessing mental health treatment, medical care, housing, food, and clothing.
- Develop individual service plans for each client.
- Coach clients in developing age-appropriate social skills, money management, personal hygiene, and job readiness.
