Occupational Therapist Home Care

About This Role

This role involves providing occupational therapy treatment to patients in their homes, focusing on rehabilitation and helping them navigate safety challenges. The Occupational Therapist will evaluate, plan, administer, and supervise treatment in accordance with physician referrals and organizational standards.

Responsibilities

• Evaluate patient's functional abilities and limitations to determine intervention needs
• Establish a comprehensive plan of care and treatment goals in collaboration with the patient and family/caregiver
• Complete comprehensive evaluations within established departmental time frames, identifying and prioritizing key limiting factors
• Implement individualized treatment plans, considering physical, social, spiritual, cultural, educational, and age-specific needs
• Provide skilled treatment safely, assessing ongoing effectiveness and modifying the plan of care as indicated
• Provide ongoing education to physicians, nurses, patients, families/caregivers, and the interdisciplinary team
• Document patient care according to departmental and regulatory agency standards, reflecting comprehensive assessment and progress
• Communicate pertinent patient information effectively to ensure optimal care follow-through
• Exhibit adaptability in caseload assignments, scheduling, and staffing shifts, assisting in other patient care areas as needed
• Provide appropriate direction and delegation to COTA, Therapy Technicians, and students, ensuring safe and quality patient care

Requirements

• Current Occupational Therapy license from the State of practice
• Current BLS certification
• Ability to become proficient with computer software applications commonly used in an automated patient care environment
• Arizona Fingerprint Clearance Card at time of hire

Qualifications

• Academic requirements of an educational program in occupational therapy approved by the appropriate governing board
