This position provides comprehensive care coordination for patients, assessing their plan of care and developing, implementing, monitoring, and documenting the utilization of resources and progress through their care. The role facilitates options and services to meet patient healthcare needs, ensuring quality clinical services and identifying/resolving barriers to effective patient care.
Responsibilities
- Manage individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes
- Provide assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups
- Coordinate all aspects of patient care from admission to discharge, interacting with physicians and clinical treatment teams
- Act in a leadership function with process improvement activities for patient populations
- Evaluate the medical necessity and appropriateness of care, optimizing patient outcomes
- Establish and promote a collaborative relationship with physicians, payers, and other members of the healthcare team
- Collect and communicate pertinent, timely information to payers and others to fulfill utilization and regulatory requirements
- Act in a leadership function to collaboratively develop and manage the interdisciplinary patient discharge plan
- Educate internal members of the healthcare team on case management and managed care concepts
Requirements
- Master's degree in social work or related field
- 5 years clinical experience
- Licensure in social work, professional counseling or marriage and family
- Board licensure in addictions (if providing services to substance abuse program patients)
- Working knowledge of care management, hospital and community resources
- Critical thinking skills
- Problem-solving abilities
- Effective communication skills
- Time management skills
- Ability to work effectively in an interdisciplinary team format
Qualifications
- Master’s degree in social work or related field or equivalent experience
- 5 years clinical experience
