About This Role

Provide direct support to individuals with brain, spinal cord, and medically complex injuries, assisting with daily living activities, health care, and maintaining a safe and clean environment.

Responsibilities

• Attend orientation and ongoing training; participate in monthly staff meetings
• Work scheduled hours, arrive on time, and communicate absences in advance
• Provide direct support for socialization, personal care, recreational activities, transportation, and other daily living activities
• Accurately document progress and activity, review records, and maintain confidentiality
• Assist with money management and safeguard individuals' funds and property
• Maintain professional relationships with individuals, families, and case managers, adhering to Network Customer Service Standards
• Report alleged abuse or neglect, and medical/behavioral incidents according to policy
• Accompany individuals to medical appointments and relay information
• Accurately administer and document medications and treatments if assigned
• Monitor individuals' health, document concerns, and communicate with nurses or supervisors
• Transport individuals safely in the community, ensuring proper use of safety equipment
• Perform daily and seasonal housekeeping, maintaining a neat, clean, and safe environment
• Monitor environmental safety and perform minor repairs or yard maintenance as needed

Requirements

• High school diploma or equivalent
• Must be 18 years of age
• Valid driver’s license in good standing
• Car registration and vehicle insurance if providing transportation for individuals receiving services
• All state-required training and certification completed in mandated timeframes

Qualifications

• High school diploma or equivalent required
• Six months of experience in human services preferred
