About This Role

This role involves providing direct support to individuals with intellectual and developmental disabilities, assisting them with daily activities, and ensuring their well-being within a community-based residential setting. The position focuses on fostering independence and creating meaningful relationships with those served.

Responsibilities

  • Provide direct support according to individual service/program plans, including socialization, health maintenance, medication administration, and skill development.
  • Assist with daily activities such as personal care, housekeeping, recreational activities, and community integration.
  • Accurately document progress and activity, review records, and maintain confidentiality.
  • Manage individuals' assets by making cost-effective and appropriate purchases.
  • Maintain professional relationships with individuals, families, and case managers.
  • Report any instances of alleged abuse or neglect and medical/behavioral incidents.
  • Monitor individuals' health, administer medications as assigned, and follow health care directives.
  • Prepare meals according to planned menus and individual dietary needs.
  • Attend orientation and ongoing training sessions.
  • Transport individuals into the community safely and responsibly.

Requirements

  • Must be 18 years of age
  • Valid driver’s license in good standing
  • Working knowledge of computers
  • Ability to lift up to 100 pounds occasionally

Qualifications

  • High school diploma or equivalent preferred
  • Six months of experience in human services preferred.

Nice to Have

  • Six months of experience in human services

Certifications

All state-required training(s) and certification(s) (Required)
