About This Role

Provide whole-person quality care to patients in their homes as a Licensed Practical Nurse within an interdisciplinary team. This role offers independence and flexibility while building rewarding relationships with a diverse patient population.

Responsibilities

  • Work alongside an interdisciplinary team to provide whole-person quality care to patients
  • Care for patients in their homes where they are most comfortable
  • Exercise independence and flexibility in patient care
  • Comply with all state laws and requirements for driving as part of the role

Requirements

  • Oregon Licensed Practical Nurse
  • National Provider BLS - American Heart AssociationWithin 30 days of hire
  • 2 years patient care experience in a skilled nursing facility or medical clinic
  • Proof of auto insurance

Qualifications

  • Education to meet certification, license or registration requirement.
  • 2 years patient care experience in a skilled nursing facility or medical clinic.

Nice to Have

  • Experience in Acute Care, Home Health or Public Health nursing
