Provide whole-person quality care to patients in their homes as a Licensed Practical Nurse within an interdisciplinary team. This role offers independence and flexibility while building rewarding relationships with a diverse patient population.
Responsibilities
- Work alongside an interdisciplinary team to provide whole-person quality care to patients
- Care for patients in their homes where they are most comfortable
- Exercise independence and flexibility in patient care
- Comply with all state laws and requirements for driving as part of the role
Requirements
- Oregon Licensed Practical Nurse
- National Provider BLS - American Heart AssociationWithin 30 days of hire
- 2 years patient care experience in a skilled nursing facility or medical clinic
- Proof of auto insurance
Qualifications
- Education to meet certification, license or registration requirement.
- 2 years patient care experience in a skilled nursing facility or medical clinic.
Nice to Have
- Experience in Acute Care, Home Health or Public Health nursing
