About This Role

Responsibilities

• Verify plan of care with the Registered Nurse (RN) prior to each visit
• Collaborate frequently with physician and RN to ensure patient needs are met and notify them of changes in patient condition
• Teach patients and caregivers self-care techniques
• Check vital signs
• Review all medications and perform a medication reconciliation at each visit
• Perform specialized procedures ordered by physician (e.g., foley catheter change)
• Prepare equipment and materials for treatments using aseptic technique
• Complete documentation timely and accurately

Requirements

• One year of clinical LPN experience
• Current LPN license specific to the state(s) assigned
• Current CPR certification
• Valid driver's license with reliable transportation and liability insurance

Qualifications

• One year of clinical LPN experience
