Provide direct, in-home skilled nursing care to adult and pediatric patients, utilizing the nursing process to assess, plan, implement, and evaluate patient conditions.
Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Provide skilled nursing care/services in accordance with prescribed orders on each assignment
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
- Collect, analyze, and interpret data and information from health care members
- Document actual and/or potential nursing diagnoses
Requirements
- Currently licensed as an LPN/LVN in the state in which the LPN will practice
- Current TB or Chest X-Ray
- Current BLS card
Qualifications
- One year prior professional nursing experience preferred
