RN-Pediatric Nurse-Homecare

About This Role

The RN-Pediatric Nurse-Homecare will utilize the nursing process to plan, implement, and evaluate patient care for pediatric and/or adult patients in their homes. This role involves administering medications and treatments, monitoring patient conditions, and providing healthcare teaching and education.

Responsibilities

• Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiate the plan of care when directed and request from prescriber any necessary revisions as patient status and needs change
• Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervene as applicable in response to changes in patient/client condition and report such changes to appropriate physician or supervisor
• Administer medications and treatments as prescribed, as well as, monitor and document effectiveness of intervention
• Review plan of care and program protocols and identify areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements

• Current nursing license (RN/LPN) for the state of New Hampshire
• Current Health Certificate (RN/LPN) contract or state regulation
• Current PPD or Chest X-Ray
• Current BLS card

Qualifications

• One year prior RN/LPN experience preferred

Nice to Have

• One year prior (RN/LPN) experience
