About This Role

Provide individualized, one-on-one shift care to patients in their private residence as a Licensed Vocational Nurse for La Casa Health Care, making a difference in patients' lives through skilled nursing care.

Responsibilities

Deliver skilled nursing care based on physician orders and care plans
Administer medications and monitor patient progress
Provide rehabilitative and preventive care
Observe and report patient changes to the RN and physician
Educate family/caregivers on home-based care and support
Complete timely and accurate clinical documentation

Requirements

Current LVN license in California
CPR Certification

Qualifications

1 year of LVN experience preferred, new grads welcome

Nice to Have

1 year of LVN experience
Strong interpersonal skills
Independence
Critical thinking
Passion for home health nursing and patient-centered care
