Licensed Practical Nurse LPN/LVN

About This Role

This role is responsible for providing and documenting skilled nursing care to individual patients, under the supervision of a Registered Nurse. The LPN/LVN will ensure quality patient care, observe and assess patient conditions, and educate patients and families.

Responsibilities

• Deliver and coordinate quality patient care in compliance with physician orders
• Continuously observe and assess patient condition and care needs, reporting changes to supervisor and/or physician
• Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
• Participate in, implement, and update the nursing care plan
• Take appropriate nursing action based on assessment and achieve expected outcomes
• Recognize changes in patient needs and responses, implementing care to prevent or reduce risk
• Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
• Provide care utilizing infection control measures according to OSHA standards
• Educate patients and families regarding disease process, self-care techniques, prevention strategies, and nursing needs
• Maintain knowledge of nursing competencies by participating in educational programs and skill evaluations

Requirements

• Graduate of an accredited school of nursing
• Current, unrestricted state license as a Licensed Nurse in the state of practice
• Current CPR certification
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies
• South Carolina: One (1) year of pediatrics experience (if applicable)
• California: One (1) year of experience working under current nursing license (if applicable)
• Louisiana: One (1) year of experience working as a licensed nurse (if applicable)

Qualifications

• Graduate of an accredited school of nursing
• Entry level with preference for 6 months recent experience in a clinical care setting

Nice to Have

• Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
• Home health experience
