This role is responsible for providing and documenting skilled nursing care to individual patients, under the supervision of a Registered Nurse. The LPN/LVN will ensure quality patient care, observe and assess patient conditions, and educate patients and families.
Responsibilities
- Deliver and coordinate quality patient care in compliance with physician orders
- Continuously observe and assess patient condition and care needs, reporting changes to supervisor and/or physician
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
- Participate in, implement, and update the nursing care plan
- Take appropriate nursing action based on assessment and achieve expected outcomes
- Recognize changes in patient needs and responses, implementing care to prevent or reduce risk
- Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
- Provide care utilizing infection control measures according to OSHA standards
- Educate patients and families regarding disease process, self-care techniques, prevention strategies, and nursing needs
- Maintain knowledge of nursing competencies by participating in educational programs and skill evaluations
Requirements
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies
- South Carolina: One (1) year of pediatrics experience (if applicable)
- California: One (1) year of experience working under current nursing license (if applicable)
- Louisiana: One (1) year of experience working as a licensed nurse (if applicable)
Qualifications
- Graduate of an accredited school of nursing
- Entry level with preference for 6 months recent experience in a clinical care setting
Nice to Have
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
