The Licensed Practical Nurse (LPN) for the Premium Evening Program at Longwood at Oakmont coordinates and supports individualized service plans for residents in personal care, providing leadership and direction to ensure quality care.
Responsibilities
- Coordinate and support individualized service plans for community residents
- Provide leadership and direction for direct support caregivers and other community team members
- Ensure quality of care and services are provided
Requirements
- LPN license in good standing from the State of PA
- Maintains CPR Certification per policy
- Ability to work well with people and show enthusiasm and motivation
- Self-directed and able to set priorities; excellent time management skills
- Knowledge of Microsoft Office programs and medical records software
- Desire to contribute to a compassionate, resident-focused culture and effectively interact with residents, resident families, and team members
Nice to Have
- Experience in Personal Care/Assisted Living
- Experience in Dementia Care
