Licensed Practical Nurse - Ambulatory Care Advanced

About This Role

This role will provide direct and indirect patient care under the supervision of a Registered Professional Nurse or Physician, functioning as part of an interdisciplinary healthcare team to improve patient health.

Responsibilities

• Directs and supervises patient care provided by other LPNs and support personnel according to department guidelines
• Identifies practice problems and consistently seeks solutions through appropriate communication channels
• Demonstrates operational problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation
• Provides direct and indirect nursing care individualized to the patient's needs
• Collects and records relevant patient health data accurately
• Evaluates health data to identify patient problems
• Implements and documents specified interventions
• Identifies deviations from expected patient outcomes
• Adheres to approved, research-based practice standards
• Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures

Requirements

• Graduation from a Practical Nursing program
• Minimum 2 years' experience in relevant area of ambulatory practice
• Current NYS LPN Licensure as a Practical Nurse or a valid interim permit
• BLS certification with 6 months of hire
