About This Role

Provide skilled occupational therapy services to in-home patients as part of a Medicare-certified home health agency. Work collaboratively within an interdisciplinary team to assess patient needs, plan interventions, and help older adults achieve optimal outcomes in a comfortable, at-home setting.

Responsibilities

  • Provide occupational therapy for in-home patients
  • Adhere to the individual patient's needs and goals as addressed in the plan of care
  • Assess patient needs based on illness or injury
  • Plan appropriate interventions
  • Work as part of an interdisciplinary home care team including RNs, PTs, SLPs, MSWs
  • Assist patients in achieving optimal outcomes

Requirements

  • Graduate of accredited occupational therapy curriculum
  • Current license as an occupational therapist from the Commonwealth of PA or application made for same
  • Minimum of 2 years of experience as an occupational therapist
  • Valid driver’s license
  • Use of an automobile in good working condition with evidence of insurance per PA State guidelines
  • Maintain CPR Certification per company guidelines
  • Familiarity with Microsoft Office 365 software
  • Ability to use electronic charting software and mobile devices
  • Able to work independently
  • Able to deal effectively with all levels within the organization
  • Passionate about person-centered care with a commitment to excellence

Qualifications

  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
  • Minimum of 2 years of experience as an occupational therapist
