Home Health Field Physical Therapist

About This Role

This role is for a dedicated Home Health Field Physical Therapist who evaluates, develops, and implements treatment programs for patients in their homes to help them maintain independence. It involves clinical direction for assistants and managing multidisciplinary care in accordance with federal and state guidelines.

Responsibilities

• Evaluate and develop specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy
• Provide the appropriate level of clinical direction for Physical Therapy Assistants
• Plan, implement, manage and evaluate the provision of both professional and ancillary home health services (direct care and case management) to ensure all patient needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines
• Manage the activities of the multi-disciplinary team providing care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Develop, implement, and carry out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team
• Interpret agency policy to patients/families
• Review patient visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits
• Inform scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

Requirements

• Bachelor's Degree Physical Therapy
• Physical Therapist State of Florida License
• 1 year Physical Therapist experience
• BLS (Basic Life Support)
• Driver's License (Florida driver's license is required within 30 days of hire)

• Bachelor's Degree Physical Therapy
• 1 year Physical Therapist experience
