About This Role

Join our team as a resident-centered Physical Therapist to provide holistic care to older adults within our award-winning campus.

Responsibilities

• Implement residents’ personal care plans
• Focus on residents’ strengths, preferences, and preferred routines
• Provide comfort, companionship, and belonging to residents
• Work with the interdisciplinary clinical team to address resident needs
• Complete documentation to meet reimbursement and regulatory requirements

Requirements

• Valid and current state Physical Therapy license
• Ability to complete documentation for reimbursement and regulatory requirements

Qualifications

• Degree and training must meet current Medicare and state requirements

Nice to Have

• Therapeutic experience in older adults across multiple diagnostic groups
