Home Care Physical Therapist

About This Role

Provide in-home physical therapy care to patients, evaluating their needs, developing individualized treatment plans, and promoting independence. Collaborate with an interdisciplinary team to deliver high-quality care in compliance with professional standards.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

Requirements

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible
• Mandatory Certifications: First Aid, AED, & CPR Certifications
• Ability to travel to patients’ homes within the assigned geographic territory

• Physical Therapist Program Graduate
• All levels of experience are encouraged to apply, new graduates welcomed. Home health experience is preferred.
