About This Role

Provide in-home occupational therapy services as a Per Diem Occupational Therapist, developing individualized treatment plans and educating caregivers. This role offers flexibility and autonomy with a focus on patient care.

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Provide 1:1 treatment
  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
  • Educate family members, home health aides, and other caregivers on proper therapy techniques
  • Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients
  • Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention
  • Provide timely and accurate documentation for each visit
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
  • Be willing to treat geriatric patients

Requirements

  • Occupational Therapist Program Graduate
  • Licensed Occupational Therapist in the state of New Jersey or eligible
  • First Aid Certification
  • AED Certification
  • CPR Certification
  • Ability to travel to patients’ homes within the assigned geographic territory
  • Strong communication skills

  • Occupational Therapist Program Graduate
