This role is responsible for processing and maintaining patient health information within the electronic medical record system for ambulatory clinics and hospitals, ensuring accuracy, timeliness, and compliance with regulations.
Responsibilities
- Verify, import, prep, scan, and maintain patient health information in the electronic medical record accurately and timely
- Validate completeness of medical record content through document review and notify providers of incomplete status
- Review electronic medical records to ensure compliance with rules, regulations, and company policy
- Identify and assign deficiencies to appropriate providers to complete minimum documentation requirements
- Monitor incomplete medical record status and notify providers of pending and delinquent records
- Monitor work queues for indexing documentation into the electronic health record
- Document and transcribe appropriate health information in the patient health record
- Create new encounters and orders to accommodate patient health information as appropriate
- Communicate routinely with physicians, office staff, and other health care facilities
- Respond to requests for health information following established facility standards, state, and federal guidelines
- Process the release of health information/medical records for continuity of care purposes
Requirements
- High school diploma or equivalent
- 1 year relevant experience
