Health and Wellness Director

About This Role

Lead the health and wellness department at an Agemark Senior Living community, ensuring compassionate, person-centered care for residents and supporting the wellness team's daily performance. This role involves developing and implementing service plans, maintaining compliance, and fostering a positive environment.

Responsibilities

• Engage with residents and support staff in daily care activities, leading by example
• Develop and implement personalized service plans in line with physician orders and resident needs
• Serve as the key liaison between residents, families, staff, and healthcare providers
• Maintain accurate records and service plans to meet state regulations
• Provide direct care when needed and supervise nursing services to ensure safety and quality
• Train, mentor, and guide wellness staff; conduct in-services and promote professional growth
• Evaluate prospective residents’ care needs and suitability for the community
• Ensure accurate and safe management of medications
• Respond promptly to resident emergencies with proper documentation and follow-up
• Identify at-risk residents and implement proactive safety plans
• Lead programs promoting health, education, and preventive care to enhance residents’ quality of life

Requirements

• Active, valid RN or LVN license for the applicable state
• Minimum 1 year experience working with seniors
• Minimum 2 years management experience leading a team
• Strong knowledge of medications, dosages, and potential side effects
• Tech Savvy; comfortable using essential software tools

Qualifications

• Minimum of 1 year working with seniors
