Lead the health and wellness department at an Agemark Senior Living community, ensuring compassionate, person-centered care for residents and supporting the wellness team's daily performance. This role involves developing and implementing service plans, maintaining compliance, and fostering a positive environment.
Responsibilities
- Engage with residents and support staff in daily care activities, leading by example
- Develop and implement personalized service plans in line with physician orders and resident needs
- Serve as the key liaison between residents, families, staff, and healthcare providers
- Maintain accurate records and service plans to meet state regulations
- Provide direct care when needed and supervise nursing services to ensure safety and quality
- Train, mentor, and guide wellness staff; conduct in-services and promote professional growth
- Evaluate prospective residents’ care needs and suitability for the community
- Ensure accurate and safe management of medications
- Respond promptly to resident emergencies with proper documentation and follow-up
- Identify at-risk residents and implement proactive safety plans
- Lead programs promoting health, education, and preventive care to enhance residents’ quality of life
Requirements
- Active, valid RN or LVN license for the applicable state
- Minimum 1 year experience working with seniors
- Minimum 2 years management experience leading a team
- Strong knowledge of medications, dosages, and potential side effects
- Tech Savvy; comfortable using essential software tools
