About This Role

Lead resident care and wellness programs at a premier assisted living and memory care community, ensuring high-quality individualized care and compliance with regulations. Oversee care planning, medication management, and provide mentorship to caregivers and med-techs.

Responsibilities

• Lead resident care and wellness initiatives, ensuring high-quality individualized care
• Oversee care planning, medication management, and ongoing health assessments
• Supervise and support caregivers and med-techs, holding the team accountable to excellence
• Hold on-call support for caregiver scheduling
• Partner with families, physicians, and staff to create a collaborative approach to resident well-being
• Provide staff training, performance feedback, and mentorship to promote growth and confidence
• Ensure compliance with state, federal, and company regulations
• Serve as second-tier on-call support, ensuring continuity of care during after-hours needs

Requirements

• Arizona Caregiver Certification or LPN license in good standing, or 4 years of related experience
• First Aid/CPR certification
• At least 2 years of experience in memory care and medication administration
• Strong leadership skills with the ability to coach and hold others accountable
• Excellent communication and problem-solving abilities
• Proficiency with Microsoft Office and other computer applications
• Valid AZ Fingerprint Clearance Card (or ability to obtain within 20 days of hire)
• Strong knowledge of AZDHS regulations

Qualifications

• At least 2 years of experience in memory care and medication administration, or 4 years of related experience if lieu of certification/LPN.
