Full Time Hospice MSW Social Worker

About This Role

Join AdvisaCare Home Health and Hospice as a compassionate MSW Social Worker/Volunteer Coordinator, serving patients and families in the Tri-City County areas. This role involves providing psychosocial and spiritual support to enhance the lives of individuals with life-limiting illnesses.

Responsibilities

• Perform initial psychosocial, emotional, spiritual and bereavement assessments
• Assist in the development and implementation of goal-directed care plans
• Provide care under the direction of a physician
• Participate in IDT conferences to observe and assess conditions affecting the patient and family
• Document all patient/family visits, telephone contacts, and referral actions timely, accurately, and concisely
• Conduct reassessments of patient/family needs and counseling as required
• Provide crisis intervention and individual or family counseling when indicated
• Play an active role as a care advocate for the patient/family unit
• Develop and maintain contact with appropriate community agencies and services
• Provide ongoing counseling related to issues of death and dying to the patient and family as needed
• Participate as a member of the Bereavement Team as assigned
• Adhere to all Hospice policies

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• Valid Driver's License, car insurance and reliable transportation

Qualifications

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)

Nice to Have

• One year of experience in Home Health Care
• Hospice experience
