About This Role

Educate patients and their families about diabetes self-management and nutrition guidelines, develop educational plans, and serve as a resource for healthcare professionals. Participate in the assessment and continuous quality improvement of the diabetes education program.

Responsibilities

  • Educate patients about diabetes self-management and evidence-based nutrition guidelines in individual or group settings
  • Assess the needs of patients and their families, formulate an educational plan, implement, evaluate, and revise the plan
  • Function as a resource to the patient, community, and other health care professionals
  • Coordinate ongoing patient care and support with appropriate members of the care team
  • Provide education, training, and support related to diabetes technologies
  • Plan and conduct continuing education programs in diabetes and general nutrition for allied professionals and the general public
  • Participate in the assessment, planning, implementation, and evaluation of the diabetes education program at the Center, including continuous quality improvement

Requirements

  • Bachelor’s degree in Nutrition Sciences or Dietetics
  • Washington Dietitian Certification or Washington Nutritionist Certification upon hire
  • National CDR Registered Dietitian - Commission on Dietetic Registration upon hire
  • Certified Diabetes Care and Education Specialist (CDCES) within 1 year of hire
  • 4 years of nutrition experience
  • One thousand hours of diabetes education experience
  • Experience and comfort with teaching and public speaking

Qualifications

  • Bachelor’s degree in Nutrition Sciences or Dietetics
  • 4 years of nutrition experience, preferably one year in ambulatory care setting or community health, and 1000 hours of diabetes education experience
