COE Case Manager

About This Role

As a COE Case Manager, you directly impact patient lives by promoting long-term recovery in addiction treatment services, coordinating with clinical and medical teams, and helping establish a therapeutic environment.

Responsibilities

• Assess new referrals for the appropriate level of care
• Coordinate services with counselors and medical teammates
• Determine need for therapeutic medical, psycho-social and psychiatric evaluations
• Monitor cases by verifying patient attendance, observing, and evaluating response to treatment, advocating for needed services, obtaining additional resources, intervening in crises, and providing universal support
• Communicate patient progress through weekly interdisciplinary meetings
• Assist with discharge plans and aftercare referrals
• Provide resources to other team members
• Participate in required annual training

Requirements

• Associate's degree in Human Services or related field
• Valid/Current Driver's License

Qualifications

• Associates or better in Human Services or related field
• Entry level with preference for previous experience in case management and/or utilization review

Nice to Have

• Previous experience in case management and/or utilization review
• Bachelor’s degree in the Human Service field
