As a COE Case Manager, you directly impact patient lives by promoting long-term recovery in addiction treatment services, coordinating with clinical and medical teams, and helping establish a therapeutic environment.
Responsibilities
- Assess new referrals for the appropriate level of care
- Coordinate services with counselors and medical teammates
- Determine need for therapeutic medical, psycho-social and psychiatric evaluations
- Monitor cases by verifying patient attendance, observing, and evaluating response to treatment, advocating for needed services, obtaining additional resources, intervening in crises, and providing universal support
- Communicate patient progress through weekly interdisciplinary meetings
- Assist with discharge plans and aftercare referrals
- Provide resources to other team members
- Participate in required annual training
Requirements
- Associate's degree in Human Services or related field
- Valid/Current Driver's License
Qualifications
- Associates or better in Human Services or related field
- Entry level with preference for previous experience in case management and/or utilization review
Nice to Have
- Previous experience in case management and/or utilization review
- Bachelor’s degree in the Human Service field
