About This Role

Lead and support a multidisciplinary team of nurses and therapists for a home health agency, ensuring high-quality, compassionate geriatric patient care and regulatory compliance.

Responsibilities

• Lead and support a high-performing team of RNs, LPNs, therapists, and home health clinicians
• Oversee patient care planning, case management, and clinical outcomes
• Ensure compliance with home health regulations and accreditation standards
• Serve as a liaison with hospitals, physician practices, and referral partners
• Deliver exceptional customer service to patients, families, and community partners
• Maintain accurate documentation and utilize EMR systems effectively

Requirements

• Registered Nurse (RN) with active Florida license
• Prior Home Health experience
• Strong leadership and communication skills
• Knowledge of geriatric care best practices

Qualifications

• Prior Home Health experience required

• HCHB (Homecare Homebase) experience
