Certified Home Health Aide (CHHA)

About This Role

This role involves providing compassionate, hands-on, end-of-life care to patients in their homes, working as part of a dedicated team. The Certified Home Health Aide will also support family members and caregivers, navigating the complexities of their journey with unwavering clinical skill and empathy.

Responsibilities

• Participate and work with a team to deliver hands-on, compassionate, end of life care
• Support patients, family members, and other community caregivers
• Navigate the complexities of the end-of-life journey
• Provide specialized care to the terminally ill
• Read and follow written instructions and document care given
• Maintain a calm manner when in a patient's home

Requirements

• Meets the training requirements of the State
• Successfully completed a state established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of S 484.36(b)
• At least 18 years of age
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill
• Satisfactory references from previous employers
• Self-directing with the ability to work with little direct supervision
• Secure with issues of death/dying
• Provides a calm manner when in a patient’s home
• Has empathy for the needs of the ill, injured, frail and the impaired
• Demonstrates tact, patience and good personal hygiene
• Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order

• Proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau
