About This Role

This Family Nurse Practitioner role provides comprehensive, community-based primary care to adult patients in Hampton Roads. The position focuses on prevention, chronic, and acute conditions, developing personalized care plans, and participating in quality initiatives.

Responsibilities

• Provide evidence-based care to adult patients, focusing on prevention, chronic, and acute conditions
• Collaborate with patients and care teams to develop personalized care plans respecting diverse backgrounds
• Perform physical exams, diagnostic testing, and prescribe medications using onsite labs and pharmacy
• Lead efforts in health promotion, disease management, and participate in organizational quality initiatives
• Maintain necessary certifications, licenses, and documentation, including EMR proficiency and team-based care experience

Requirements

• 3-5 years of primary care experience
• Board certification
• Proficiency with Electronic Medical Records (EMR)
• Experience in team-based care
• Passion for serving underserved populations
